Schedule A is required in any year you choose to itemize your deductions. The schedule has seven categories of expenses: medical and dental expenses, taxes, interest, gifts to charity, casualty and theft losses, job expenses and certain miscellaneous. Each of these categories has different requirements and limitation on the amount you can deduct.

If you are self-employed, it’s likely you need to fill out an IRS Schedule C to report how much money you made or lost in your business. This form must be completed and included with your income tax return if you had self-employment income.

When filling out the FAX-A-TAXSM online questionnaire, you will have the option to upload any additional required documents in Section 1. If you are Self-Employed or a Sole Proprietor, you will have the option to upload your Schedule C Affidavit in Section 8. Should you need to attach a Schedule A Affidavit, you will have the option to upload it in Section 8 as well.
You will need:
- Tax Information such as Forms W-2, 1099, 1098, 1095.
- Social security cards or ITIN letters for all persons on your tax return.
- Picture ID (such as valid driver’s license) for you and your spouse.
- Please complete sections 1 – 8 of this form below.
- You are responsible for the information on your return. Please provide complete and accurate information.
